Medical Information Kits
The fire department is excited to implement a free program that allows residents to provide information regarding medical history, conditions and prescription medication in times of great distress by offering Medi-Facts kits. Medi-Facts is a medical information storage kit that makes personal health records easily accessible to emergency medical personnel.
How the kit works:
- Fill in the medical history information sheet. If you can, have your doctor review your information.
- Fold the medical history sheet so information faces inside and the “Important Medical Information” label is visible.
- Slide the sheet into the magnet and stick to refrigerator door; keep it highly visible and away from other items.
- Place the provided alert sticker on or near the exterior door of your home. This will alert Queen Creek Fire to the location of your medical history.
Packets can be picked up at the Town of Queen Creek Development Services Building or any Queen Creek fire station.
